Hi,

There are some broken links in TOC of the CHM file (left bar of the chm window).
These bugs are present since revision 12427 (at least).

How to repeat:
Try expand the chapters "MySQL 5.1 Frequently Asked Questions" and "Errors, Error Codes, and Common Problems" and click on a sub-section.

The latest version of the CHM is correct, but there is no guarantee that similar problems will not reappear. 

Unfortunately the size and complexity of our CHM builds is now approaching sensible limits for the format, and I'm looking into ways of simplifying the process to get more accurate builds.
