Description:
during code-review was one abort path in DBLQH discovered that did not release
a commit-ack-marker.

furthermore, it was also discovered that during node-failure-handling
the LQH's could be added several times to the marker-record
which could lead to node-failure when the array overflowed.

How to repeat:
new testprg (for bug#41295)

Documented bugfix in the NDB-6.2.17 and NDB-6.3.21 changelogs as follows:

An abort path in the DBLQH kernel block failed to release a
commit acknowledgement marker. This meant that, during node
failure handling, the local query handler could be added
multiple times to the marker record which could lead to
additional node failures due an array overflow.

(Fix appears in 6.3.21 rather than 6.3.20 per email from Jörg.)
