• Create SQL test table: execute "myodbc_catastrophic_failure.sql"
  mysql.exe --user=root --password=******** --host=localhost < myodbc_catastrophic_failure.sql
• create a virtual directory /MyODBC mapped on current directory on your local IIS Web Server
