I have noticed the duplicated menu entry problem too and I haven't figured out when it happens. I can't reproduce it the way described

Try double click to open MySQL Workbench on your Desktop icon. Open a connection. Try to see table properties with right button. All right till here. With your MySQL Workbench opened, come back to your Desktop and try double click to open MySQL Workbench in Windows again. The MySQL Workbench won't open twice, but the last "session opened" will show up. Open a new connection. Try to see table properties with right button again. Thank you.
