Description:
After removing a index from Table Editor, Index name and Index Columns are left over. And if I try to delete the Index name, access violation exeption occurs.

How to repeat:
In 'Schemata' tab, right click on a table and select Table Editor.
In 'Column and Indices' tab, select a column.
In 'Indices' tab, click 'plus' button and add new index at the 'Index Column' area.
Click 'Apply changes'
Click 'Execute'
In 'Indices' tab, click 'minus' button and drop index at the left side of the tab.
Click 'Apply changes'
Click 'Execute'
The index name is still in the box.
Click the box and delete the index name.
-> Access Violation exeption

Suggested fix:
Remove the index name together when applying the change.
