Description:
Feature request to add a custom header or footer to the ERB printouts.

How to repeat:
Print an ERB diagram.  Observe no useful information to determine what you are printing.

Known Workaround: Add a text field to the ERB document and add your information there.

Suggested fix:
Add header/footer options to the Page Setup options.  Elements that would be nice:
* Last modified date
* Name (from File > Document Properties)
* Version (from File > Document Properties)
* Author (from File > Document Properties)
* Project (from File > Document Properties)

Fixed as of the upcoming MySQL Workbench 6.0.4 release, and here's the changelog entry:

A page header and footer was added to the "Print to File..." output. 

A workaround option was to add a text field to the document and add your
information there.
