We finally determined that the mysql user account that was used by the application didn't have permissions to the mysql.inventory table. Therefore, the plugin results couldn't get reported to the server. Of course, this has been a common user error when installing the agent and I overlooked its relevance to the plugin as well. I'm not sure if this warrants a mention in the readme or not, but I thought that I'd pass along the info to save someone else some troubleshooting time.

Good feedback - thanks! Mark / MC - could you make sure this is mentioned in both the plugin README and the documentation?

How to repeat:
See Description.

It's already in the documentation - fourth item in the opening bullet list of requirements. I've made a small clarification to ensure that it is the connecting user authorization required here.
