Description:
Looks like "MySQL installation type" widget not cleared before filling

How to repeat:
1. New server instance
2. Click "Next" and get to "Operating system" step
- MySQL installation type has 8 items (OS Linux, by default)
3. Click "Back", "Next"
- MySQL installation type has 16 items
4. Click "Next", "Back"
- MySQL installation type has 24 items

...and so on :)

Suggested fix:
Clear "MySQL installation type" widget before filling it with set of items

An entry has been added to the 5.2.22 changelog:

In the Create New Server Instance Profile wizard, on the Operating System page the MySQL Installation Type drop-down listbox was not populated correctly. Although all items were initially correctly displayed, if the Back button was clicked, and then Next clicked to return to the page, all the items in the list were duplicated. Each time this process was repeated the original list items would again be duplicated.
