Description:
after a server crash, InnoDB eventually asserts in fil_io() complaining about accessing a page outside of the known limits of the tablespace.

How to repeat:
very hard to reproduce and extremely unlikely to happen in a customer
environment, but it does happen in my stress and recovery testing.
