Description:
When agent is ungracefully restarted (crash, kill -9) the log will show DuplicateAgentUuidException for couple minutes.

How to repeat:
kill -9 agent processes and start (re)agent immediately. See agent log.

Suggested fix:
Not sure. Maybe we should just document it.
