Thank you for the report. Unfortunately we can not find cause of this without knowing version of server and what Cisco ANM does with it. Only similar bug is bug #8996 which was fixed years ago. Please report this in Cisco bug tracker or send us information which we can use to find what happens: accurate version of MySQL server and full error log file for start.
