Thank you for the bug report. I couldn't repeat with the script attached just error messages that table doesn't exists. Please provide complete dump file of the tables involved. Thanks in advance.

I've loaded your test database into MySQL 5.0.79 on Windows XP and executed the sript 10+ times in a row from QB 1.2.17 on the same XP. No access violations. So I suspect server is (part of) a problem here. Please, upgrade server and check again. Latest server version is 5.0.84.
