Description:
Currently, we have only one way to add multiple mysql instances to a group. By looking at the name on the "Add to group" popup (On UI->Settings->Manager servers)

There are times when you need to see more information to distinguish among servers with the same name.

It would help a lot if you could use the current check-boxes next to each server name, and then click on "Add to group", select a group name, and you are done.

How to repeat:
1- Have the name of 3 mysql instances be the same.
2- Add each of them to a specific group based on whether they are online or offline.
3- You would be force to either rename them, or add them all to each group and then remove the "extra" servers from the wrong groups.

Suggested fix:
Use the already present check-boxes and add a "add to group" link, which then will show you the different group names you have. (and allow for adding the same server to multiple server groups as well)
