How to repeat:
1. Install MySQL 5.0.56
2. Uninstall MySQL 5.0.56
3. Install MySQL 5.1.24
4. Add new user using Query Browser 2008

does not look like an InnoDB bug. The error msg is from server ACL part.

Do you use same data directory for 5.1 server which you used for 5.0 server?

No - MySQL Server 5.1.  Don't know why I didn't get an email to go with your comment although I was subscribed to this bug, is that a problem your end our with our spam filter?

Incendently did a full delete of "c:\program files\mysql\mysql server 5.1" today and still expereincing issues starting\ adding user re: mysql.servers error.

Need to be cleared on upgrade sometimes.
