I have some questions: 1) did you upgrade from 5.0 to 5.1 and didn't use mysql_upgrade? 2) would you provide a reproducable procedure that you tested?

This seems to be related to bug #48157 which was fixed in 5.1.43. Please test with 5.1.43 or higher.
