Thank you for a problem report. First of all, please, use newer version, 4.1.18. Have you tried to install as local Administrator? Do you have any firewall in your system? Other versions of MySQL already installed?

This also happens on the latest version. The installation creates a service with the wrong executable name. The executable name set in my case was mysqld-max which I do not have. Checkout the Component Services entry for MySQL and check the properties.
