I have on my XP the SP2 installed and I was unable to repeat the behavior reported. Are you tried uninstall the driver using Control Panel Add/Remove program or the driver installer ? and then to re-install it?

Few days ago I even downloaded SP4 for MS SQL Server 2000 from Microsoft site and installed it.This might have modified some files and hence the bug.I think now you can repeat the problem I am having. note: I can create mysql dsn without any problem only when I first create a new or test already created SQL Server 2000(SP4) dsn.
