Description:

"MySQL Startup Item" would not install and gave no reason for the failure.  Since I have only 
recently installed 10.2.8, the directory /Library/StartupItems/ did not exist.  As soon as I created 
the directory manually the problem was solved.

Remove the directory and try installing

Suggested fix:
mkdir

Thanks for the information. I would find this very strange,
as this is the default location to install Startup Items.
I have asked my contact at Apple for confirmation. If this
is really the case, I would say it's a bug in Mac OS X...
