Description:
Enhancement request:  Change references to syntactic elements to
also be hypertext links to the definitions of the referred-to 
syntactic elements.

For example, make references to the GRANT statement be hypertext
links to section 4.3.1 (or directly to the specification of the 
syntax of the GRANT statement); in the GRANT syntax specification,
make the reference to "priv_type" be a link to the definition
of priv_type.

Suggested fix:
Since keywords are already marked up specially (they're a 
different column), making each reference a link could probably 
be significantly automated.  (By chance are SQL snippets
already marked differently than others?)
