Description:
Using the mysql-standard-4.1.12-apple-darwin7.9.0-powerpc package, the startup item installed by MySQLStartupItem.pkg appears broken.

Running the script returns:
  # ./MySQLCOM start
  Starting MySQL database server
and hangs indefinitely.

How to repeat:
- Install mysql-standard-4.1.12-apple-darwin7.9.0-powerpc.pkg and MySQLStartupItem.pkg on Mac OS 10.4.2.

$ cd /Library/StartupItems/MySQLCOM/
$ ./MySQLCOM start

Have you tried with newer 4.1.14 version (you can download it from http://dev.mysql.com/downloads/mysql/4.1.html)?
