Description:
In workbench on MySqlModel tab, try attaching a script to Synchronization. It will end up attaching it to Forward Engineering and will not run it before/after sync.

It messed up my Forward engineering, I spent hours trying to figure out. I gave up. 

I discovered the issue only now when I needed to attach a script to synchronization...  

How to repeat:
in Workbench on MySql Model Screen
expand Sql Scripts
create a new script or edit an existing one
on the edit screen change Synchronization to "Bottom of Script", observe the value of Forward Engineering (say it is Do Not Include)

Now apply and close your edit screen.

Click on the same script open edit screen again, notice the values for Synchronization and Forward Engineering. 
Synchronization will be "Do not Include" and
Forward Engineering will be "Bottom of Script"

Suggested fix:
  Attaching synchronization action selection to the proper item
