Description:
The downloaded file 'mysql-5.6.21-osx10.9-x86_64.dmg' (MySQL Community Server) only includes 

'mysql-5.6.21-osx10.8-x86_64.pkg' 

but not 

'mysqlstartupitem.pkg' , 'mysql.prePane' and 'read me.txt'.

The Startup Package is also missing in the development release 5.7.5-m15.

It turns out that this is a documentation bug, in that all three packages were merged into one. The Preference Panel and Startup Item features are now options. Here's the new changelog entry:

The OS X 10.8+ binaries now bundle the "MySQL.prefPane" and
"MySQLStartupItem.pkg" tools into the main package as configurable options
instead of separate packages.

Also, the documentation has been updated accordingly. Thank you for the bug report and sorry for the confusion. The Preference Panel and Startup Item are enabled by default when executing the main installation package. However, as of OS X 10.10, the Startup Item won't function so you'll need to use launchd.
