Description:
ndbd could leak ConcurrentIndexOperations leading to 288 when performing transactions

How to repeat:
Run transactions using unique index lookups

Suggested fix:
dont leak

I've documented this for 4.1.16 - should this be in 5.0/5.1 changelogs and if so for which release(s)?
